Our commitment to your privacy
Avenmind Health Group is required by law to maintain the privacy and security of your protected health information, provide you with this notice of our legal duties and privacy practices, and notify you if a breach occurs that may have compromised the privacy or security of your information.
How we may use and share information
We may use or disclose your health information to provide treatment, coordinate care, obtain payment for services, operate our practice, or when otherwise permitted or required by law. Other uses and disclosures generally require your written authorization.
Your privacy rights

Good Faith Estimate information
If you are uninsured or choose not to use insurance to pay for your care, you may have the right to receive a written Good Faith Estimate that explains expected charges for scheduled items or services.
How to request an estimate
Contact Avenmind Health Group at (561) 858-1661 or contact@avenmindhealth.com to ask for a Good Faith Estimate. If you request an estimate before scheduling care, the practice can explain the next steps. If you schedule care at least three business days in advance, a Good Faith Estimate may be provided according to applicable requirements.

AvenMind Guide and website information
The AvenMind Guide is a conversational practice-information assistant. It can answer general questions about Avenmind Health Group’s services, telehealth, ages served, listed insurance plans, appointment process, and how to contact the office. It can offer a call or general-email handoff, but it cannot schedule care, review records, diagnose, recommend treatment or medication, or provide crisis support.
What visitors may enter
Use the Guide only for general questions such as “Do you offer telehealth?” or “How do I request an appointment?” Do not enter names with health details, diagnoses, symptoms, medication information, medical history, insurance member numbers, records, or emergency information.
Where a message goes
The current message and a limited recent conversation history are sent through the website server to the server-side assistant to generate a reply. This site does not save the Guide conversation to a browser, account, or practice database. The Guide is not a HIPAA intake channel, and visitors should avoid entering protected health information. As with any internet service, the underlying hosting and model providers may have their own operational logging and retention practices outside this site’s application database.
Website and emergency information
The information on this website, including the AvenMind Guide, is for general educational and practice-information purposes. It does not establish a provider-patient relationship and should not be relied on for diagnosis, treatment, medication decisions, or medical advice.
Please do not include urgent concerns, highly sensitive medical details, or protected health information in website messages or the AvenMind Guide. Avenmind Health Group’s website, email, and information assistant are not emergency services.
